I am very pleased to announce that we have recently earned several Community Health Quality Recognition (CHQR) Badges from the Health Resources and Services Administration (HRSA). These CHQR badges recognize health centers that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response for the most recent Uniform Data System (UDS) reporting period.

HSRA Health Center Quality Leader badge

Sam Rodgers has been recognized as a silver tier Health Quality Leader. That means our clinics have achieved the best overall clinical quality measure performance and our scores put us in the top 11-20% of all Federally Qualified Health Centers (FQHC) in the nation.

HSRA Advancing HIT for Quality badge

Sam Rodgers earned an Advancing Health Information Technology (HIT) for Quality award. This means we met all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health data to increase access to care and advance quality of care.

HRSA Addressing Social Risk Factors badge

Sam Rodgers earned an Addressing Social Risk Factors Award. This was award recognizes health centers screening for social risk factors impacting patient health and are increasing access to enabling services.

Patient Centered Medical Home badge

Sam Rodgers was also recognized as a Patient Centered Medical Home (PCMH). The patient centered medical home model is an approach to delivering high-quality, cost-effective primary care. Using a patientcentered, culturally appropriate, and team-based approach, the PCMH
model coordinates patient care across the health system.
